CVI File Upload
State Animal Health Official Use Only
This site is for use by State Animal Health Officials and their staff for the purpose of uploading CVIs for animals being exported to Illinois.
Please follow the instructions below to submit files.
- Select your state from the drop-down list
- Under Upload CVI(s), select “Click to Attach CVI(s)”
- Choose the CVI file(s) from your computer that you wish to upload and press “Open”
- Once the upload is complete, press “Submit”
- The “Reset” button can be used to cancel the action before submission
